* Effective Spring 2020, the above paper forms used to request new academic plan codes, update existing plans and inactivate academic plans will still be accepted in instances where such changes have already been fully approved.  However, any new academic programs (including degree programs, minors, certificates, etc.), program modifications, and requests to inactivate programs must now go through CIM Program Management.  Contact the Office of the University Registrar if you have any questions about CIM.
